Annual Registration Renewal 2010

The online Registration Renewal system is now closed.

Those who have not yet renewed their registration may do so by completing the paper version of the Registration Renewal Form found below.  

For those who currently hold a four-month Independent Practice certificate and wish to renew their registration, please submit your Registration Renewal Form two weeks prior to your current certificate expiry date to allow for processing time.

New Information Collection

Please note that changes to the Regulated Health Professions Act require the College to collect data that can be used for health human resources planning and information about offences that registrants have committed or findings of negligence and malpractice. All of this new information must be provided in the renewal process. Please carefully review all sections of the renewal form and guide to ensure that you are providing the most accurate information possible.
