A summary of the allegations is as follows, that Mr. Bayat (Bayfield):

• Abused a patient
• Engaged in conduct or performed an act relevant to the practice of the profession that having regard to all circumstances would reasonably be regarded by members as disgraceful, dishonorable or unprofessional
• Engaged in conduct unbecoming a physiotherapist
• Failed to maintain the standards of practice of the profession
• Performed a professional service for which consent is required by law without such consent
• Failed to keep records in accordance with the standards of practice of the profession
• Falsified a record
• Signed or issued a document containing a false or misleading statement

A summary of the allegations is as follows, that Mr. Bayat (Bayfield):

Discontinued professional services that were needed
Contravened by act or omission, the Act, the Regulated Health Professions Act, 1991 or the regulations under either of those Acts
Engaged in conduct or performed an act relevant to the practice of the profession that having regard to all circumstances, would reasonably be regarded by members as disgraceful, dishonorable or unprofessional
Represented qualifications in a manner that is false, misleading or deceptive
Practiced the profession while using a name other than the member's name as entered on the register
