Complaints or Concerns about a Physiotherapist

If you have a concern or complaint about a physiotherapist, you have the right to submit a formal complaint to the College. 

If you are not sure if you want to file a formal complaint, you can always contact the College to learn more and discuss your specific situation. The College of Physiotherapists of Ontario exists to protect the public.

How to Make a Formal Complaint

You can send your complaint to the College in a number of ways: you can use the online form to file a complaint below, submit the complaint in writing or as a recording, or you can download a complaint form to print off and then send it by email, fax, or mail.

What Information Do You Need to Include in the Complaint?

We will need:

  1. The name of the physiotherapist. Staff can help if you do not have the exact name or you can look up the name of the physiotherapist and the clinic or facility address online using the College’s Public Register.

  2. Your name and contact information. Please include your daytime telephone number and your mailing address.

  3. The patient’s name (if this complaint is about someone other than you).

  4. As much detail as possible about your concern or the event that took place. For example, what happened, who was involved, when and where it happened and what you hope will happen as a result of making this complaint.

You will be contacted by a College staff person within 2 business days.

Personal Health Information Protection Act, 2004 (PHIPA)
PHIPA governs health care information privacy in Ontario. If you are the patient, please be aware that PHIPA permits practitioners to disclose personal health information without obtaining consent in the following circumstances: 

  • if disclosure is needed to provide health care, and consent cannot be obtained quickly;
  • to contact a relative or friend of an injured, incapacitated or ill client for consent;
  • to confirm that a client is a resident or client in a facility, provide his/her location and comment on his/her general health status (unless there is an express request not to do so); or
  • to eliminate or reduce a significant risk of harm to a person.
